The St. Clair County Health Care Commission and the Healthier Together: 25 by 2025 Movement


The St. Clair County Health Care Commission is a collaborative community health alliance that mobilizes resources to meet identified health needs and promote the health and well-being of all of the residents of St. Clair County.

Since 1991, the Commission has worked to assess and address the public health needs of the citizens of St. Clair County. Since inception, Commission partnerships have increased fourfold, and have been recognized as a statewide and national model for collaboration.

In early 2018, the Illinois Department of Public Health approved the Commission's County-wide 2017 Community Health Assessment and 2018-23 Community Health Improvement Plan; and began to work with the grassroots Healthier Together Movement as part of a larger Collective Impact approach that grew out of the 2017 Health Summit.

This year the Commission is working with partners and volunteers to convene the 2019 Health Summit around a shared and long-range vision to become Healthier Together.  The Summit is designed to engage leaders across all community sectors to join the movement, learn more about the health priorities that jeopardize our county’s health, and initiate change through strategic community building.

The mission of the Healthier Together movement is to create a healthier St. Clair County and metro east area. The Healthier Together movement is 100% volunteer driven. This grassroots campaign requires a true investment by all. Governance, implementation, and accountability are shared by all members. The Healthier Together movement seeks to build a culture of Health & Wellness among the communities of Metro East Illinois and to drive St. Clair County to be within the top 25% of healthiest counties in Illinois by 2025.
